Dear Bonnie,
Prem labour is a risk with twins anyhow whether one has problems or not. 
Don't let this dr scare you into doing something you quite obviously don't 
want to do.  Many women with singleton pregnancies go into prem labour and 
that is out of your control but carrying the second twin with T18 isn't 
going to make your risk any greater.
The risk of losing twin A through a selective termination would have to be 
almost equal to that of losing 'her' through prematurity.  I would be 
seeking a second and third opinion.
