Our Rachel has a DORV large VSD and a small ASD. Her
heart condition has essentially been unchanged since
birth (at least that's what her cardiologist tells
us). She's on lasix & digoxin since 3 months of age.
Her O2 sat was great from birth to 7 months, in the
90s; it dropped to the upper 80s after her G-tube
surgery at 7 months. She is now recovering from her
first pneumonia and requires oxygen. We don't know yet
if she can be weaned off. 
At 9 months, we decided against heart surgery for
Rachel. She was doing great without it and we couldn't
make her go thru a major surgery like that.   
We did discussed heart surgery with a surgeon before
Rachel was borned (before her T18 diagnose). The
recommendation was to fix the heart within 2-3 months
of birth, otherwise there's a good chance for other
complications to develop. 
I am not sure if any of this is helpful. Hope
everything goes ok with your friend's grandchild.
