If you have no insurance and have huge medical bills, you can sometimes get
put on Medicaid.  My step-mother had quit a job and had no medical insurance
when she had a stroke. Before she passed away, her bills were over $50,000
for just one week (today would be much more, as that was 12 years ago).  My
Dad filed with Children & Families (after we pressured him to do so) and
they agreed to put her on Medicaid for that week because the bills were so
much that it put them in financial distress for that time period.
Otherwise, he would have been paying forever.
