[THIN] renamed pub app reappears with old name

We are using MFxp SP2/FR2 Sql datastore
We have renamed some published apps and removed permission to another, now
every once in awhile when a user logs in they will see the old apps. The
list will show the original app name and an app they used to have
permission to, when they click 'refresh', the list shows them what they
are supposed to see.

any thoughts?
Where is this information coming from?
Is it cached somewhere?
