After you create your New imalhc.mdb file, you should go into Regedit and
change the \\HKLM\HARDWARE\CITRIX\IMA\RUNTIME and change the PSRequired
value from 0 to 1. Then start the IMA Service. That should recreate LHC
from the DataStore.

Yes.
I think the issue is related to the LHC or the data store (MSSQL). The first
time the IMA service starts, it reads the database for users and displays
that users in the CMC (forever, until the service restarts). Users that
logon or off while the IMA service is running or not registered in the Users
Tab.
This brings up a few questions:
Are the changes registered in the LHC and written to the database when IMA
starts (or stops)?
How can you force the LHC to be written to the data store?
