[THIN] Duplicate icons in WI4

Anyone seen anything like this? We get it intermittently & have been
unable to find a pattern to it. All the published app icons are
displayed twice, and I've had reports of one of the icons working, one
not. Logging out of WI and back in usually sorts it but it seems to be
becoming more frequent.
