That's what it said in the subject line. (Akai VCR) ;-) I forget to look there sometimes too. Could be the mode switch (as Chester said) or the sensors. It some cases it can also be the carriage out of alignment or loading belts/gears bad/misaligned, or depending on the unit, loading motor support components.
