One thing everyone forgot, logic! If you hear the ballast trying to fire 
the lamp then the "ballast power source" is good, the "power on" signal 
to the ballast is good, the ballast is good, the lamp is bad.
The only reason we routinely replace both the lamp and the ballast is 
because 1. The set is in warranty AND 2. It is another 50 to 70 miles 
each way to go back. If it's out of warranty AND in town we only replace 
what it takes to get it working again.
A far as I know every one we have seen where the lamp would turn off 
then turn back on has been a bad lamp.
