Strange to have horizontal and vertical at the same time.
You need to do to the EPROM set up, and see if you can bring them into 
phase.
There is a Capacitor you can bridge in to keep it from going into shut down 
at full line voltage, but  I have always found it hard to see the Parameter 
numbers and set up with this workaround.

I would suggest to try a new EPROM before you waste too much time on it.
