Ok, I believe you and have trawled through the Regs to satisfy myself.
In practice how many Authority's out there actually bother and is it a
rubber stamping job?  It may be the case that retrospective approval is
needed for multiple sites in my area.  Also from memory to apply for
specific authorisation the views of the Police need to be sought, I can
see them being less than impressed if they are approached on a site by
site basis unless a blanket approach is acceptable to DfT.

Finally, does anyone have a pro-forma setting out what exactly needs to
be submitted to the Regional Office?

We outlined this in a TSG Open meeting about a year ago, but here is
more=20
detail.

In TSR&GD the centre of the signal head (or amber) is specified as 4000=20
max (see Diag 3000) . This value  will usually be exceeded by the=20
'additional' head when double-in-line heads are used - particularly
since,=20
for maximum conspicuity, there is usually some separation applied betwen

the 'additional' and the 'standard' head. As I see it, therefore,
specific=20
authorisation is required to exceed this height.=20

DfT have indicated that the authorisation procedure for such head=20
arrangements should in most cases be straightforward. Nevertherless,
they=20
are concerned over what they consider to be an unecessary proliferation
of=20
double-in-line heads, so they may ask more searching questions about=20
particular sites.
