I don't know your particular requirements but you could try a single ended
to differential balun from picosecond labs. They have a bandwidth to
somewhere around 10GHz. Miteq (www.miteq.com) makes some very good broadband
amps with low noise figues. I don't know if they have a single to
differential amp though.
