Charles,
Treat the model as you would any other component in the circuit. Use the
.lib, or .subckt call as others have mentioned. Terminate unused pins in
50ohms (assuming this is the Zo), and be careful of the grounding on both
sides of the black box. Sometimes the black box has an internal connection
to ground, and sometimes they don't.
You will need either a .lib or a .subckt call from the main program. If the
model file is located within the same directory that you are running the
simulation you can use relative directory calls. If it is not you will need
to point Spice to the directory (i.e. c:\xx\yy\zz.txt).
It's probably a good idear to hook up the black box by itself first and fire
it up to see what kind of performance you get prior to connecting it to the
rest of the circuit. I've seen just about every shape and size of connector
model, so if you need additional information feel free to contact me offline
