The symptoms match CMIR violation. CMIR TRULY extending to either
supply rail is atypical. Only amplifiers with specially designed FEs
actually extend to one rail the other or both. There are many
amplifiers particularly FET based that get close if you ignore
performance degradation near the rails. Read the data sheet very carefully.
