[samscommunity] New Student Registration begins July 21

MESSAGE FROM ST. ANDREWS MIDDLE SCHOOL:


New Student Registration will begin on Monday, July 21.  In order to make the 
enrollment process a smooth transition, please have the following documents 
available at the time of enrollment:

1.  Copy of previous academic year's report card

2.  Birth certificate (if registered by a parent) or Proof of guardianship (if 
conducting non-parent enrollment)

3.  Social Security Card

4.  Immunization Record - All students must be in compliance with SC 
immunization requirement laws for school attendance.  The law states that 
students must have the following immunizations upon enrollment:
          Hepatitis B - 3 shots                                      OVP - 4 
shots
         DTP or DtaP - 4 shots                                     MMR - 2 shots

If you are transferring to St. Andrews Middle School from out of state, you may 
request a 30 - day waiver from the school nurse at the time of enrollment.

5.  Proof of Residence for School Zone or Residency Affidavit

     Two documents needed                                                       
                 from Registrar's Office
      Examples:  water, electric, cable bill, lease agreement
