Your 1 and 3 are correct.
2 is correct only in a very limited sense. A SAC member doesn't have to
sign a waiver if he doesn't attend a SAC event that requires one.
So to lose your membership: be a SAC member AND attend a SAC event
requiring a signed waiver AND refusing to sign that waiver.
