In our area the check comes to us direct from the undertaker. It is listed on
the initial estimate of funeral expenses/contract that the family signs prior
to the funeral with the undertaker. I get a form for my taxes from the funeral
home every year. I have a feeling that perhaps this is a more common setup in
Catholic parishes, since none of the Protestant churches for which I've worked
had the same arrangement.
