Yes, I sent that article to this person along with a lot of other resources.

But he can't even seem to remember to save them as a template or make the 
corrections we ask for.   It's very frustrating, especially for Christina.

Here is a link to an excellent article on creating accessible forms in Word
using Jaws.

This is from the Freedom Scientific web site.  The directions work well for
Word forms, but this may not necessarily address other difficulties with web
based forms.
