Actually, thinking of it, what exactly do you need to trace patchpanel connections? If you have two 24-port patchpanels, and you create 24 RJ45 ports and 24 110Block ports on each, name them like rj45-1..rj45-24 and 110b-1..110b-24, then connect 110Block ports between 2 patchpanels as you want, you'll have some kind of mapping between ports of the patchpanels. Of course, you need to assume that rj45 and 110b ports with the same number are corresponding.
