Richard I don't have a clue about the history either, but I know they made flat 
4x5 carriers because I have it.  IF in a bind and couldn't locate one that 
easily it could easily be cut out of a flat aluminum sheet or steel sheet for 
that matter.  It didn't look that hard.  Mine uses a couple of rivets I think 
on the top and bottom of the negative to keep it from slipping around.

With some basic metal working tools if you have a carrier for a different 
negative size for the outside pattern, it would also give you the center needed 
for the 4x5 center.  IF you can't do it, also any machine shop should be able 
to do it in no time.  One with a CNC machine it would be a matter of minutes.

Granted it may be more cost effective to wait till you could find one, but it 
would be an option if in a bind.
