One more possible source (if I may).

Were you using rapid fix? On a few occasions I have had undiluted Rapid fix 'precipitate out' when it has gotten well past the "best-by" date, and finding out I do not have enough for immediate use, I have been guilty of mixing a small amount to make up the required volume with only the safelight as illumination.. Bad move.... some fine precipitate has landed in the required aliquot of fixer.. and was not 'observed' until the white light was switched on. But I must admit that I have never experienced the fine precipitate 'clinging to the film' to cause a problem similar to yours.
