Again, Claudio Bonavolta has been kind enough to post this on
his web site at:

I am on a rather slow dial-up so this is a great convenience to
me beside allowing anyone who wants the chart to get it without
having to write for it.
I scanned the entire instruction sheet. Its very old and has
some wrinkles I couldn't fix but is quite readable. Its on an odd
size sheet so its scanned as two overlapping pages.
The chart covers the standard colors in the basic kit sold by
SpoTone but has brief descriptions of the extra colors and of
Spotoff. Spotoff, as near as I can tell, was an Iodine type bleach.
