OK, I've gone through all the books looking for this and updated the xml (and committed to svn). I'll output new versions and update the test area tomorrow...

In the case of "i.e." I would argue that it's usually wrong to use a
comma because "i.e." translates as "that is" and so the comma
separates a verb from its complement which is a no-no.

We should add an entry in the Manual of Style. Any objections to the status quo?

None--let's avoid unnecessary commas! :-)
