Hello Bryan and listers,

    Why show those boxes if no one will need to put information in them?
It's one more thing one must muck through when filling out the form, but if
the information has all ready been supplied why bother someone with it?

why is it so important not to show the extra box?
i usually have a box for comments below all of the info i want to collect.
