[phpa] Zeus + FastCGI

Is anyone using PHPA with Zeus and PHP as a FastCGI module? I would like to
but get this error (on stderr no less) on each page request:

SERVER_NAME unset for request - unable to activate PHPA (pid xxxx)

FastCGI doesn't define the same environment variables as Apache.
