[phpa] Version 1.3.4 released for Linux, FreeBSD, OpenBSD and Solaris

Hi,

A new version of PHPA has just been released. This is a minor update, and
improves the handling of PHP crashes, and reduces the liklihood of the SHM
cache remaining in the 'pending reinitialisation' state.

The behaviour of phpa_cache_admin when listing processes has been enhanced
to show whether or not an 'active' process is really alive or dead.
