[phpa] Update on [non] problem reports for 1.3

Just an update on the problem reports for 1.3. Both turned out to not be
issues with the new release.

The user report of SERVER_NAME being undefined was because both the latest
and older extension were being loaded, and behaviour is then undefined

The user report of being "unable to add registry item" was because domain
keys were being added multiple times.

Please email suspected problems to me or bugs@xxxxxxxxxxxxxxxxxxxxx rather
than to the list to keep traffic down.
