[phpa] Survey on segfaults

Hi,

Occasionally I get reports of people experiencing occasional segfaults
and I'd like to gather statistics about system configurations to see
whether any common trends are evident.  These are likely to be due to
php itself or other features, but I'd like to keep aware of the state
of things.

If you are aware of segfaults in your logs then if you reply with the
following details then that would be appreciated.

1. OS and version
2. PHP version
3. configuration line for PHP
4. Is safe mode enabled?
5. Does your code use array_walk/array_map/array_reduce
