[phpa] PHPA and php cgi/cli

Unexpectedly the current release of PHPA does now work with cgi/cli php,
although only with the file cache and not shm cache.

Provided that php is not running as a daemon, with a parent process of 1,
PHPA will not try and create or access any shm cache and so not interfere
with any phpa shm cache also in used by apache.
