[phpa] Re: HTTPD fails to start/spawn children

        I should have mentioned both items before.  One of the annoying parts 
of 
the failure is that there was nothing output to the Apache error log. 
Also, when it was working before the failure neither the shm_user nor 
shm_group were explicitly set.  After cruising through the mailing list 
archives I saw this as a common response to failure reports so I 
explicitly set them to no avail.
