[phpa] 1.2p4

I've installed the new 1.2p4 beta in my production server. :)
It's a heavily loaded server...

The release notes and config notes explain the changes for this release.

Please let me know how you get on. This should cope better if there are
server crashes, although at the moment you may get a very noisy log if it
detects crashes, and the shm cache may get disabled without being reenabled
for a while. So you'll get less acceleration basically. But then hopefully
you won't get server crahes.
