I guess you didn't delete the drive partition and recreate it before 
doing the format.

there is a good chance that there is nothing physically wrong with the 
drive, it could just be that the partition table is messed up.

of course another option is that the BIOS setting are not the same as 
when the drive was partitioned originally. Generally the BIOS typically 
has 3 cyl-head-sec schemes (algorithms) to use, if the formula is 
changed after the partitioning is done then strange & BAD things can 
happen including files being over written, drive suddenly corrupted, 
etc. You would be best off to start from scratch, delete all files, 
remove the current partitions, restart the machine and enter the BIOS, 
have the BIOS AUTO-DETECT the drive (most mobo's in the last 6 years can 
do this) accept the autodetect drive parameters. reboot, partition the 
drive as needed, then format and install your OS.
