The association itself is the only part that would need to be added or modified within the registry. Of course, it would need to point to the portable app which would negate the association's portability, but the app itself would still remain portable and would leave the registry alone. You can have an association point to a portable app that resides on a USB stick, a CD or DVD or even a network share but it would only function as long as the USB stick, etc. was attached/mounted and using the same exact volume letter. The first time it's not there, you'll be asked to associate the filetype with something else and the portable association will be broken.
