Laurence,
    Do these errors mention any filenames other than explorer.exe?  For 
instance, do any of them show shmedia.dll, ntdll.dll or some other filename 
like that?  It might be part of the error window's title or perhaps within a 
"More Info" button or something like that.

    The offset numbers don't tell us anything other than where the code 
happens to exist within memory during that one instance, so they will change 
just about every time.
