But, with Mailwasher, it's a separate app that needs to be run prior to
downloading my mail with Outlook. Is this the same? Or, does it work
directly with the mail client, as a proxy, where it doenloads the mail and
drops to Outlook?

Hi Troth,
I think it is better than MailWasher. It sorts out the spam I believe based 
on the collective experience of all the owners of 0Spam. After less than a 
week my spam is down 98%. I highly recommend it.
They also have a huge forum listed at their website.
