with this info:

Instead of double-clicking on the downloaded hotfix zip file, 
right-click and choose Run As Administrator! It will unzip correctly. 
You then need to go to the unzip directory and double-click on the 
extracted file (no Run As Administrator needed or possible).

Nowhere in the download instruction email does Microsoft mention this, 
and Error 40 isn't covered in any of their knowledge base articles. Grrr!

That lack of Error 40 info was galling. I searched for it too. another annoying 
Vista "feature" I only just discovered is the rt click/Run As Admin option. My 
iTunes updates used to always fall into an endless loop before.
now I got the files to unzip ... into the ether. So I went spelunking around 
until I found them. 
some hotfix it's asking for a restart! I thought hotfixes installed on the fly 
"hot." whatever!
I think I'm cured now. I saw the sign-off & sign-on countdown.
