Some do. Makes it annoyingly hard to tell what's on and what's off.
As for the cross-linked files, it's POSSIBLE to have 2 files with the
same name, but it's not correct. Check the name very carefully to
make sure that it is indeed identical. Try copying both files to
another folder to see if you get a prompt to overwrite; if so, the
files indeed have the same name. Otherwise, there is simply a
difference that you haven't noticed.
