There are quite a few possibilities, but basically, if your AV is set
to scan email, and/or set to scan files before they are
opened/executed, then you don't need to do it manually, unless you're
feeling paranoid.

If your AV program supports scanning email, but is not set to do that
automatically, and is not set to automatically scan files before they
are opened/executed, then a single manual scan *should* be sufficient.
When an AV program or other utility examines such attachments, they
usually do not "look" nested.
