CSV file

You might want too try exporting the address file from Outlook to a CSV 
file if possible and see the format of the headers, in other words the 
fields coming from Thunderbird have to be identical to the OUTLOOK 
fields in order for the OUTLOOK import to be in the right columns.  I am 
not sure if OUTLOOK import actually requires the header titles to be 
identical, it may accept them if they are in the proper sequence.
