What programs have you been using other than CWShredder?

Here's the list of software that I've been using to clean up and to
prevent recurrence. Be sure to use the latest version/update of all
programs:

Full antivirus scan (all files) with a current sig file.

Microsoft Antispyware (beta expires July 31), explore the various
tools also.  Do quick scans until it's clean, then run a full scan.

SpywareBlaster Blocks a lot of sites. Explore the various tools.

WinsockFix Cleans up the TCP/IP stack; I suggest using only in severe
cases.

Unbrand.VBS Resets IE and OE to the default customization.

IE-SpyAd Blocks a lot of sites, in a manner different from
SpywareBlaster.

HijackThis Shows you all of the settings, toolbars, BHOs, etc., for IE
(these affect Windows Explorer also) so that you can remove them if
you think that they might be baddies.

I haven't used these two much since I decided the MS ASW seems to work
well: AdAware and Spybot Search & Destroy.  Note that both of them
have a number of tools.

Once the system seems to be thoroughly clean, and seems to be running
smoothly, I install XP SP2, to tighten things down even further.
