I'm currently getting something similar with my server, even after
replacing the floppy drive, trying other disks, new motherboard and
cables. It's had me stumped.
Now you gave me an idea. To test the operating system theory, get a
clean DOS boot disk that's never been in this machine before. Set the
tab to write protect if it's not already there. Put it in drive A: and
restart in DOS. After booting, type
FORMAT A:
When the inevitable error message appears, replace with a blank test
disk and see if DOS formats it. If it does, there's a gremlin in the OS.
