Hi John,
I've seen this sort of thing from time to time. As far as I am aware, there
are four things you can try.
1) Turn the machine off (physically off, not a restart). Wait about 10
seconds, turn it on and try again
2) Try booting into DOS and formatting the disk from there
3) Replace the floppy drive (to determine if that was the problem)
4) (and typically my first approach) toss the floppy disk (not the drive) in
the bin and get a new one.
They are so unreliable compared to other storage options these days that I
avoid using floppy disks wherever possible.
