Wish I could give more concrete answers but he just did his a month ago 
so he's still in the learning mode as well.  His was only 24 pages and 
he calls it an instruction manual rather than book, but technically it 
is a small book.  He's not going the traditional route of selling as he 
does his promoting on his website, where there is a link to the book 
from there.  He doesn't want to mass market it as it is meant for a 
select target audience, most of whom visit his site.  However, by the 
sound of the Lulu FAQ's page, it sounds like Lulu gives you an ISBN and 
allows you to market with other places, such as Amazon and Barnes and 
Noble.  Lulu prints and ships the book as needed by these retailers. The 
distribution process (in the FAQ's) sounds straight forward but of 
course it takes doing the process once before you know for sure.  Wish I 
could give you more definite answers to your questions.

Xlibris is another option as Scott mentioned.  A friend of mine got her 
book done there but there is a fee.  I don't know anything more about 
that service other than that.
