Forgot to add, does the cardinality of the column matter, what if the
only possible values are "Y" and "N", what if 99.9% of the rows are all
"Y"? Does the index still prevent a deadlock? 

What are the internals on this, anyone know?

Yes, unindexed FKs can absolutely cause deadlocks.  I recently had a
problem with this happening frequently for an application called
Khameleon.  After adding FK indexes, the deadlocks vanished.
