Are you using EMC snapview product for the backup ? It uses copy-on-write algorithms to create multiple copies of database snapshots and manipulates pointers to keep multiple copies of the database. Snapview uses a SaveArea to store the prior images/points to the blocks and I guess, in theory, it is possible to run in to this situation if you run out of free space in the save area. Specifically, probability is higher during restore as the rate of changed blocks increases.

something in the NAS or OS logs.  The Unix admins told me they didn't see any 
errors on the NAS, but maybe I'll ask them to double check.

writes to an FS cache and gives the all clear to lgwr but the write
never makes it to disk?
