I found it!  The problem (feature?) is with
SQL*Navigator.  I should have mentioned that I was
using it.  It executes alter session on connect to
change the NLS_DATE_FORMAT to whatever is configured
in the user options (which I changed recently).

Testing from SQL*Plus yields the correct format.

goose chase.  He's been caught!
