Peter,

I would look for /var/log (or any other place dep, on the OS) to make sure
there are no issues at hardware. You may get 15196 for hardware related
issues as well.

I don't see the version or platform info in your email. There are few issues
in 10203 and earlier (and 2TB size limit)  and you might want to look for
the patch for 15196.  Btw how big are the LUNs?
