1.Is this cloned environment?
2.Did it use virtual node when clone?
3.the result uname -n is?(it should be ORACLEDR according to log files)
4.select * from fnd_nodes and sure the node name your environments on is
exists.
please check your CONTEXT_SID.xml file,and sure there have no original
node_name.
you can execute below sql and reexecute autoconfig in db tier and app tier
and retest this issues.

sqlplus apps/appspassword
>EXEC FND_CONC_CLONE.SETUP_CLEAN;
