Just checked I do not see CUSTOM top defined there , whats the procedure to add
it ...do we need to bounce the apache? or the instance?

The most likely reason is formservlet.ini file does not have Custom Top
defined, specially if servlet mode is being used.
Review this file and verify if your custom top is defined correctly in
formservlet.ini file in $APACHE_TOP/jserv/etc
If you are using Socket mode, then ensure that Custom Top is sourced as well
when you source the environment.
