 As John said, you can usually focus right on the objective lens using the "-" 
diopter setting.  Chances are it is dirt/tears on the lens, but it could also 
be scratches in the lens coating.  

More infrequently it is a misalignment of one of the wheels or levers:  check 
that the astigmatic slide is all the way in, that the mag lever and filter 
wheels are in their detents, and that the small pupil wheel is clicked back in.

If the lens is dirty, you may need a solvent to fully remove the oils from the 
lens.  We have had the most success with acetone and cotton balls.  We have had 
scratching when using lens cleaning tissue!

We have a Topcon TRC50X camera that has a reflection dead center of the
color and red free image.  It appears to be increasingly worse.  The
reflection appears to be from the flash of the camera.  Objective lens
issue?  Thoughts?

Ahh, the infamous "Topcon edema". Most likely a dirty lens. Dial in the
minus diopter, then rack focus back all the way, then turn the viewing
light up to full to visualize the culprit.

Agreed.
Also, the "alignment option", which we used to call "white dots", will show up 
on mono chromatic setting. Disable it prior to shooting an FA.
