Dolby website shows distribution method for major networks, but not FOX:

DD5.1 into video signal (& FOX planned to change to embedded DD5.1):

This more recent set of articles describes each network, incl. embedded
method for FOX....however, some stations use baseband switches
(hence Lt/Rt problem), whereas some stations use modern transport stream 
switches.
